1. Services Provided

Lifted Aesthetics and Wellness, LLC offers medical and aesthetic treatments including, but not limited to:

• Botox® and other neuromodulator injections

• Dermal fillers- Hyaluronic acid 

• Sclerotherapy

• Laser treatments

• Microneedling, medical facials, chemical peels, regenerative therapies

• Skin rejuvenation treatments

• Weight loss therapy and hormone replacement therapy

• Functional medicine and aesthetic consultations

All services are provided by licensed professionals in accordance with Florida law and under physician supervision where required.

2. Medical Disclaimer

• The content on our website and social media platforms is for informational and educational purposes only.

• It is not intended as medical advice or to replace consultation with a qualified healthcare professional.

• Results vary between individuals; no outcome is guaranteed.

• All patients must complete a medical intake form and provide truthful information before receiving treatment.

3. Appointments, Payments, and Cancellations

Booking

Appointments may be booked online or by phone at (386) 999-1366.
A valid credit card is required to hold all appointments.

Deposits

Certain services require a non-refundable deposit to secure your appointment. The deposit will be applied toward your service total at checkout.

Cancellations and No-Shows

• We require at least 24 hours’ notice for appointment cancellations or rescheduling.

• Late cancellations or no-shows may result in a forfeited deposit or a fee equal to a percentage of the scheduled service cost.

• Repeated no-shows may result in suspension of booking privileges.

Payments

Payment is due at the time of service. We accept major credit/debit cards and other methods as offered through our payment processor (e.g., Square or Aesthetic Record).

4. Refunds and Returns

Due to the nature of medical aesthetic services:

• All treatment sales are final.

• No refunds will be issued for completed services or partially used treatment packages.

• Product returns are accepted only for unopened items within 14 days of purchase, subject to inspection and receipt.

5. Photography and Marketing Consent

With your consent, we may take before-and-after photos for medical documentation or marketing purposes.

• All identifiable images will only be used with your written consent.

• You may withdraw consent at any time in writing by contacting us at liftedaestheticswellness@gmail.com.

6. Website Use and Intellectual Property
